Steps To Enrollment
1. Contact us! — Call or e-mail our Intake Coordinator to discuss your child’s needs:
2. Intake Packet — Once you have decided to start services we will send you a packet to complete. Some of the information requested includes a list of prior treatments, current school, and other background information that will be needed :in order to complete a treatment plan. Within that packet is a health insurance form that enables us to contact to your health insurance carrier on your behalf to request authorization for funding of your child’s assessment. Insurance companies require an Assessment and Treatment Plan prior to authorizing services. Please return the completed enrollment forms, along with:
Prescription for ABA from your Primary Care Physician
Copy of the diagnosis
Copy of insurance card (Front and Back)
Copy of the latest IEP or IFSP if There is One
Copy of previous assessments (Diagnostic / Psychological / Behavioral, and Speech and Language)
3. Assessment Process — The Behavior Analyst assesses the skills that your child exhibits so that we can measure the progress made throughout the program. L2L utilizes both standardized and curriculum based assessments to establish baseline skills. The assessment processes includes: parent interviews, observations, and direct assessments. Children that are exhibiting maladaptive behaviors will also have a Functional Behavior Assessment (FBA) to address all areas of need. Once the assessment is complete this report will be submitted to the appropriate funding source who can then authorize the commencement of services.
4. Starting Services — As we get to know your child and family throughout the assessment we will be building his/her treatment team. Each treatment team consists of one Clinical Supervisor and one or more Behavior Interventionists depending on the intensity of services.
